You finish a glass of red wine or a plate of leftover pasta and within an hour your face flushes, your head starts to pound, and your stomach turns. You’ve been told it’s stress, or maybe a food allergy, but allergy tests come back clean. If this pattern sounds familiar, histamine intolerance may be the explanation you’ve been missing.

Histamine intolerance is estimated to affect roughly 1% of the population, though many clinicians believe that figure significantly undercounts the true prevalence — simply because the condition is so rarely part of a conventional diagnostic workup. Its symptoms are wide-ranging and easy to attribute to something else, which is precisely what makes it so frustrating to live with.

What Histamine Intolerance Actually Is

Histamine is a chemical your body needs. It regulates stomach acid production, helps transmit nerve signals, and plays a central role in immune responses. When mast cells — specialized immune cells distributed throughout your tissues — detect a threat, they release histamine to trigger inflammation and help clear the problem. That’s the system working as it should.

The trouble starts when histamine accumulates faster than your body can break it down. Under normal circumstances, an enzyme called diamine oxidase (DAO) handles the bulk of histamine clearance in your gut. When DAO activity is insufficient, dietary histamine passes into circulation instead of being neutralized, and histamine intolerance symptoms follow.

Those symptoms can affect nearly every system in the body: headaches or migraines, skin flushing, hives, nasal congestion, heart palpitations, low blood pressure, digestive cramping, bloating, and diarrhea. The Gut Connection You Can’t Ignore

The most important thing to understand about histamine intolerance is that it rarely exists in isolation. In most cases, something is wrong in the gut — and that gut dysfunction is both reducing your ability to clear histamine and increasing the amount your body is producing.

DAO is produced in the lining of the small intestine. When the intestinal barrier is compromised — whether by SIBO, dysbiosis, inflammatory bowel conditions, or increased intestinal permeability — the cells that produce DAO are damaged, and enzyme output drops. Research from the University of Barcelona has shown that intestinal dysbiosis may contribute to mucosal inflammation and, in turn, favor the development of a leaky gut with a subsequent reduction in DAO enzymatic activity.

The microbiome complicates the picture further. Not all gut bacteria are neutral bystanders — many bacterial species produce histamine as a metabolic byproduct. A 2022 study in *Nutrients* found that patients with histamine intolerance had significantly lower levels of health-supporting bacteria like Faecalibacterium prausnitzii and Ruminococcus, and a higher abundance of histamine-secreting species including Staphylococcus, Proteus, and Clostridium perfringens. This means the gut isn’t just failing to clear histamine — it’s actively producing more of it. Even people without a true DAO deficiency can develop histamine intolerance symptoms when histamine-secreting bacteria dominate their microbiome.

This gut-histamine connection is why addressing histamine intolerance through the gut is so important. Healing the intestinal lining, rebalancing the microbiome, and resolving any underlying SIBO or dysbiosis can restore DAO production and meaningfully reduce histamine burden — something a supplement alone cannot achieve.

High Histamine Foods: What to Avoid and Why

Histamine doesn’t form in food randomly. It builds up through bacterial fermentation, aging, and the breakdown of proteins over time — which is why freshness matters so much. The longer a food has been sitting, fermenting, or curing, the more histamine it contains. Cooking doesn’t help: once histamine forms, it’s heat-stable and cannot be destroyed.

The foods most reliably high in histamine include:

  • Fermented and aged foods: red wine, beer, champagne, aged cheeses (parmesan, blue cheese, cheddar), sauerkraut, kimchi, soy sauce, vinegar, and most fermented soy products
  • Cured and processed meats: salami, pepperoni, bacon, deli meats, smoked fish
  • Certain fish: especially canned tuna, mackerel, anchovies, and sardines — fresh fish is lower, but histamine accumulates rapidly after the catch
  • Histamine-liberating foods: these don’t contain high histamine themselves but trigger its release from mast cells — citrus fruits, strawberries, tomatoes, avocado, spinach, eggplant, chocolate, and alcohol

A practical low-histamine approach focuses on eating fresh. Fresh meat cooked and consumed immediately, most plain vegetables (excluding those mentioned in the last bullet point above), fresh herbs, rice, quinoa, and most cooking oils are well-tolerated. Johns Hopkins notes that these foods are not inherently harmful and should only be eliminated when clearly associated with symptoms — the goal is identification of personal triggers, not permanent restriction.

Leftovers are worth special attention. Histamine continues to accumulate in cooked food as it sits in the refrigerator, so meals that seem perfectly safe when freshly prepared can become problematic the next day.

Why DAO Supplements Aren’t the Whole Answer

DAO supplements are widely available and actively marketed for histamine intolerance. They can be genuinely useful — particularly as a short-term bridge while underlying gut issues are addressed, or as an “insurance policy” before a meal containing known triggers. A pilot study published in the Journal of Food and Nutrition Research found that oral DAO supplementation produced significant improvement across all histamine intolerance symptoms during the four-week supplementation period.

The catch is what happened next. Once subjects stopped taking the supplement, symptoms returned. That rebound tells the story: DAO supplements suppress the symptom without touching the underlying problem. If your DAO production is low because your gut lining has been damaged by dysbiosis or permeability, adding enzyme from outside the body manages the consequence while the cause continues untreated. A recent review in Biomolecules reinforced this directly, noting current challenges including limitations in supplementation efficacy and calling for approaches that address the root causes of DAO deficiency.

Beyond the rebound issue, DAO supplements don’t do anything about histamine that originates within the gut from histamine-secreting bacteria — a significant source for many people. Supplements won’t rebalance a dysbiotic microbiome, repair intestinal permeability, or resolve SIBO. Those problems require targeted interventions.

Natural compounds like quercetin and vitamin C can support the process by stabilizing mast cells and reducing the rate of histamine release. But like DAO supplementation, they work best as adjuncts to a root-cause strategy, not replacements for one.

What a Root-Cause Approach Looks Like

An integrative workup for histamine intolerance typically includes testing for SIBO and gut dysbiosis, an assessment of intestinal permeability, and — where appropriate — measurement of serum DAO activity. Identifying what’s driving low DAO production or elevated histamine load allows treatment to be targeted rather than generic.

In practice, this often means a combination of a temporary low-histamine diet to reduce the immediate burden, targeted gut healing protocols (which may include specific antimicrobials for SIBO, gut-supportive nutrients like L-glutamine, and carefully chosen probiotic strains that don’t produce histamine), and gradual reintroduction of foods as gut health is restored.

The good news is that histamine intolerance is frequently reversible once the gut issues driving it are resolved. Many patients find that after a period of treatment, they can tolerate foods they previously had to avoid entirely.
